Adding an Initial license via Independent Application Checklist

Please hold onto syllabi and course documents, in the event that additional information might be requested.  Please follow instructions below. 

NOTES:

Candidates who already hold an Initial or Professional license in another field have two options for adding an Initial license in Moderate Disabilities, Instructional Technology, and English as a Second Language.  The options are:

    1. Complete the entire approved program (both the degree and the practicum). Framingham State University will endorse the candidate's application for the license, if requested.
    2. Apply directly to the Massachusetts Department of Elementary and Secondary Education, without the practicum and without the FSU endorsement, after completing the master's degree program.  Please be aware that:
    • ESL, Moderate Disabilities, and Instructional Technology licenses require an internship in place of the practicum. Please see the DESE internship template at http://www.doe.mass.edu/educators/InternshipLetter.pdf.
    • the following licenses require a competency review by DESE:
          • Moderate Disabilities

          • Visually Impaired

          • Deaf & Hard-of-Hearing

          • Severe Disabilities

          • General Science, 1-6
            (Competency Review or Subject Matter MTEL testing option)

          • History, 1-6
            (Competency Review or Subject Matter MTEL testing option)

          • Instructional Technology

          • Foreign Languages
            (For those languages where there is no Subject Matter MTEL offered)

          • Library 

Applying for Your License

Order an official copy of your transcript from Framingham State University. On the request form, indicate that you want the transcript sent to your home address, after you've completed your degree. Once you receive the transcript, do not open it.

Order and submit an official, unopened copy of your transcript for your bachelor's degree to:

NOTE:  If you already hold a Preliminary license, DESE has your bachelor's degree transcript on file.  You do not need to submit a copy of your undergraduate transcript.

Go to the Licensure area at the Massachusetts Department of Elementary and Secondary Education web site. Log on to your ELAR profile area and select the option to Add an Educator License. Follow the prompts to update your education information. Then, use the following to apply for your Initial license:

When asked which route you are following, select an option which does not require completing an approved program. Complete the remaining screens to apply for your license.

If you have had a name change (e.g. marriage, divorce, other legal proceeding) while in (or after) your licensure program or the name on your undergraduate transcript is different than your current name, you must submit a name change form with appropriate documentation of the name change to the DESE.

Please print and submit a copy of your current license.

Please be sure to label all items with your license number.

To expedite the licensing process, please make sure you have:

  • set up your ELAR profile,
  • applied for the license,
  • paid for the license,
  • labeled all items with your license number

Please send your transcript and employment verification form to the address below.  If necessary, please include a name change form with appropriate documentation of the name change.
