The information submitted through this form is provided to the Framingham State ID office from Student Government Association and Student Involvement and Leadership Development. The student submitting the information will be granted access to Club Room 2 in the McCarthy Center by utilizing the ID card reader. It is the responsibility of club officers to forward the submission form to authorized club members of the space.

Club Room 2 is for the purposes of recognized FSU club/organization business.
Any misuse of the space and its materials tied to my FSU ID could lead to, but is not limited to, suspension of club privileges and/or judicial charges.
I am responsible for the care, use, and return of borrowed materials from the Student Government Association (SGA) office.
SGA and Student Involvement and Leadership Development reserve the right to suspend access during any investigation into misuse of the space.
